We accept most medical insurances. Below is a list of medical insurance providers with which we are contracted, however this is not a guarantee of coverage for your specific plan. If your provider is not listed below, but your plan allows for out of network benefits, we may be able to bill to your insurance on your behalf.
Please be advised that some medical insurances only cover medical eye care visits and not routine eye exams. If you are coming for a routine eye exam, please contact your insurance company to determine if you will be covered in our office.
Our office is an out-of-network provider for most vision benefit plans. If you have a vision benefit plan (VSP, EyeMed, BlueViewVision, CignaVision, Davis Vision, Spectera), we can help you to submit for out-of-network reimbursement, if your plan allows. If you have a vision benefit plan, you may also be eligible to have a routine eye exam through your medical insurance, so we advise you to contact your medical insurance to determine if you may be covered in-network with your medical plan.

Out of network: Ambetter, AmeriHealth, Humana, Wellsense, vision benefit plans (VSP, EyeMed, BlueViewVision, CignaVision, Davis Vision, Spectera).
We accept all major credit cards, cash, and personal checks. We also accept Care Credit. Contact our office to learn more.
We do not want payment to stand in the way of anyone’s proper eye health care, so payment plans can be arranged on a case by case basis. If you do not have insurance and are in need of financial assistance, you may also contact the Conway Area Lions Club, which provides assistance for those in need of eye care.
You can use the attached form to request having prior records sent to our office, or having our records of your care sent to another provider. If you are coming to our office for the first time, this form is included in the new patient packet above. If you have extensive records from another provider, we encourage that you have those sent to our office prior to your visit, so that our providers may review in advance and provide the best care during your visit.
