Whether you are brand new to Conway Eye Care and Coos Eye Care, whether it has been a while since you have been in our offices, or whether you are a current patient being seen regularly, we value you and the opportunity to care for your eyes! While our office is accepting new patients, the demand for eye care in the valley has grown exponentially. We want to remind our established patients to always schedule your next appointment well in advance, to avoid delays in your care. For new patients or patients who have not been seen in several years, we do have longer wait times for an appointment, but we also have a cancellation list, if your schedule allows you to be flexible for openings with short notice.
If you are a current patient and would like to schedule your next visit, please click “Request An Appointment” at the top of the page. If you are a new patient or have not been seen in more than three years, please first print, complete, and return the patient forms below, and once received we will contact you to schedule your visit.
Maximize Your Time With Us
We value you and your time ! To maximize your time during your appointment, our patient intake forms are available here. If you are new to our office or if it has been three years or more since your last visit, these forms need to be completed before your visit is scheduled. If you are a current patient, we encourage you to update these forms any time you have had significant changes in your contact information or health history, to save time when in office. Please print out the forms below and return them to our office as soon as possible, either by mail, in person, or via email. Thank you and we are looking forward to seeing you soon!
We accept most medical insurances. Below is a list of some of the medical insurance providers with which we are contracted, however this is not a guarantee of coverage for your specific plan. If your provider is not listed below, but your plan allows for out of network benefits, we may be able to bill to your insurance on your behalf, or you may be able to self-submit for reimbursement.
Please be advised that some medical insurances only cover medical eye care visits and not routine eye exams. A routine eye exam is a preventive care or wellness visit, like your annual physical with your primary care provider. It does not include the management of any eye conditions aside from a prescription for glasses and/or contact lenses. If you are coming for a routine eye exam, please contact your insurance company to determine if your visit will be covered in our office under your medical insurance.
Our office is an out-of-network provider for all vision benefit plans. If you have a vision benefit plan (for example VSP, EyeMed, BlueViewVision, CignaVision, Davis Vision, Spectera, Superior Vision), we can help you submit for out-of-network reimbursement, if your plan allows. Even if you have a vision benefit plan, you may also be eligible to have a routine eye exam through your medical insurance, so we advise you to contact your medical insurance directly to determine if you have coverage for a routine eye exam in our office through your medical plan.
Insurances Accepted: Anthem/Blue Cross Blue Shield, Aetna, Cigna, Harvard Pilgrim, Health Plans, Medicare Part B, some but NOT ALL Medicare Advantage Plans, NH Healthy Families, United Healthcare
Out of network: Ambetter, AmeriHealth, Humana, WellSense, MaineCare, all vision benefit plans (eg. VSP, EyeMed, BlueViewVision, CignaVision, Davis Vision, Spectera, Superior Vision).
We accept all major credit cards, cash, and personal checks. We also accept Care Credit. Contact our office to learn more.
We do not want payment to stand in the way of anyone’s proper eye health care, so payment plans can be arranged on a case by case basis. If you do not have insurance and are in need of financial assistance, you may also contact the Conway Area Lions Club, which provides assistance for those in need of eye care.
You can use the attached form to request having prior records sent to our office, or having our records of your care sent to another provider. If you are coming to our office for the first time, this form is included in the patient forms above. If you have extensive records from another provider, we encourage that you have those sent to our office prior to your visit, so that our providers may review in advance and provide the best care during your visit.
